Aztech Labs 14.4 Data+Fax+Voice Driver. This is for the modem/soundcard that came with a bunch of Packard Bells. One chip says Crystal CS4231A-KL and another says Rockwell RCV144ACF-P and another says Aztech AZT2316R. FCC is I38 or 138, then MMSN841. The sticker on the back says AZSC53428581. It worked for me with Win98.
